We collect FOUND stuff: love letters, birthday cards, kids' homework,
to-do lists, ticket stubs, poetry on napkins, telephone bills, doodles -
anything that gives a glimpse into someone
else's life. Anything goes...

Obviously a one-year-old can't write..the fact that it says 'over-sized heart' makes me think she was making some sort of dead baby book.
I agree - it looks like a poor diagnosis. 'big heart' is cute, 'over-sized heart'... not so cute.

clearly this is from some little girl playing doctor. i would guess this is a diagnosis is for an imaginary patient or a doll. I would suppose after the patient left the hospital the "chart" would be discarded
