Actually, all kidding aside, mental hospitals do snap a quick polaroid of patients upon intake usually, to put in their file. Perhaps this guy refused to get out and they were so flustered that they decided to snap his picture in the car, while the therapist, with her coffee in hand, sat in the back seat attempting to coax him out.
